New Student Enrollment Forms

Welcome to Christian Victory Academy! We are excited to join your team in education!

Step 1: To begin your application for enrollment, please fill out the following contact form so that one of our administrators can contact you to make an appointment. If you would prefer, you may call us at 407-281-6244.

Step 2: Please fill out the following forms. The forms are available as interactive PDFs here on the website, and may be filled-out online, BUT must be printed and signed for CVA's records.

  2. School Transfer Request

  3. Curriculum List (Only fill out for homeschooling courses)

  4. Affirmation of Commitment & Disclaimer

  5. Code of Conduct & Dress Code

  6. Likeness Release Form

    In addition, please bring the following to your appointment

  7. Legal Guardianship Documentation (only required in custody cases, such as adoption, divorce, etc.)

  8. Birth Certificate (copy)

  9. Original Blue Immunization Form

  10. Payment* - The application fee ($200) is due with the application.
    (Please make check or money order payable to: Christian Victory Academy)

*If your Application for Enrollment at CVA is not approved, a non-refundable fee of $50.00 will be retained. Once your student begin school, the entire amount paid is non-refundable.
