Step 2: Please fill out the following forms. The forms are available as interactive PDFs here on the website, and may be filled-out online, BUT must be printed and signed for CVA's records.
As of 8/1/13, Firefox updated their Adobe Reader Plugin. Unfortunately, the plugin has a glitch. Until Firefox releases a new update, please open all forms in any other browser, since they all seem to be working properly. Some options are IE, Chrome, and Safari.

    In addition, please bring the following to your appointment

  8. Legal Guardianship Documentation (only required in custody cases, such as adoption, divorce, etc.)

  9. Birth Certificate (a copy that CVA can retain)

  10. Original Immunization Form

  11. Payment* - The application fee ($200) is due with the application.

*If your Application for Enrollment at CVA is not approved, a non-refundable fee of $50.00 will be retained. Once your student begin school, the entire amount paid is non-refundable.
