AFFIRMATION OF COMMITMENT and DISCLAIMER

We or I (hereafter referred to as "We" and "Our")* hereby certify that the answers on this application are true and correct. We understand that Christian Victory Academy of Central Florida, Inc. (CVA) is a Christian organization, as put forth in our doctrinal statement. We are in agreement to home school our child(ren), and we understand and agree to the following:

  1. CVA, it's directors, staff, or members, shall NOT be responsible for the education of our child(ren); rather, we are responsible for their education.
  2. We are responsible to choose and maintain our own curriculum, and all curriculum costs are our responsiblity. Due to Florida Deptartment of Education and accreditation requirements, CVA reserves the right to approve or reject any curriculum selections to ensure fulfillment of minimum standards.
  3. If we choose to enroll our child(ren) in any on-campus classes, we agree that we are in a partnership with CVA, and will take resposibility to ensure that our child(ren) complete all assigned homework.
  4. We will meet and maintain all requirements of, and abide by, the policies and procedures of CVA.
  5. Negligence of these responsibilities may result in a penalty fee or dismissal from CVA without refund. Reinstatement is not guaranteed, and is conditional upon reaffirmation of these standards, a favorable decision by the CVA Board of Directors, and payment or repayment of applicable fees.
  6. We release and hold harmless CVA, it's directors, staff, and members from any and all claims for loss, damage, and/or injury of any nature, resulting from CVA programs or activities.
  7. We agree / disagree with the Statement of Faith, and will, to the best of our ability, uphold it's principles in our child's (children's) schooling.

We (I), hereby acknowledge and understand that Christian Victory Academy of Central Florida, Inc. is in no way part of, nor are we sponsored by, Calvary Baptist Church. We (I) hold harmless Calvary Baptist Church, it's members, or staff from any responsiblity between Christian Victory Academy of Central Florida, Inc. and us (me).
