New Student Enrollment Forms

In order for your family to become part of Christian Victory Academy of Central Florida, Inc.(CVA), it is necessary to complete the following forms, and bring them with you to your appointment. You must call or e-mail CVA for an appointment at (407) 281-6244 or [email protected]. If you are planning on taking only on-line courses, then please e-mail or call for special instructions.

The forms are available as interactive PDFs here on the website, and may be filled-out online, BUT must be printed for CVA records. This is to ensure your privacy, and to eliminate forgotten items.

*Note: If your Application for Enrollment at CVA is not approved, a non-refundable fee of $50.00 will be retained. Once your student begin school, the entire amount paid is non-refundable.
