CVA Enrollment Instructions

In order for your family to become part of Christian Victory Academy of Central Florida, Inc.(CVA), it is necessary to complete the following forms, and bring them with you to your appointment. You must call or e-mail CVA for an appointment at (407) 281-6244 or [email protected]. If you are planning on only taking on-line courses, then please e-mail or call for special instructions as you do not need to come in for an appointment.

The forms are available here on the website, and may be filled-out online, BUT must be printed for CVA records. This is to ensure your privacy, and to eliminate forgotten items.

Items 1 - 10a must be completed and turned into CVA no later than August 1st. Please e-mail [email protected] for late enrollment due to extenuating circumstances.

Enrollment Checklist

4.	Affirmation of Commitment & Diclaimer
5.	Phone Directory
6.	Code of Conduct Form
7.	Legal Guardianship Documentation (only required in custody cases, such as adoption, divorce, etc.)
8.	Birth Certificate (copy) - Required
9.	Original Blue Immunization Form - Required
Payment* - Application fees are due with application. (Please make check or money order payable to: Christian Victory Academy)

After the 3rd child, the Annual Registration Fee is waived.
